Instructions make grids less-than-pretty. Instead of repeating instructions in every row and causing awkward staggering, why not move the instructions to the <th>? above and below could become separate <p> tags or maybe a <tfoot> would look nice for belows.
This format looks great on EE's Matrix (I realize this isn't an actual Matrix, but I couldn't find a good example and it looks basically the same):
